It’s happening in bits and pieces all over healthcare. Any time a provider offers bundled prices, or bids on reference prices, or builds onsite clinics, or retail clinics, any time they do a risk contract for a population, or for instance contract with an employer to be part of a captive accountable care organization — all of these are ways of stepping outside of the Default Model. So are doctors who establish direct pay practices. In the “volume to value” move that healthcare is consumed with, it is important to note that the Default Model is a discounted fee-for-service system, which means you build your revenue stream on volume.
